Since 2004, we’ve helped thousands of seniors get the services they need. It could be help with eating, driving to medical appointments, showering and cooking, medical care, or gaining strength. We have 12 different areas of care, all with one purpose: Keeping the aging adult in their home and community as long as possible.
When you call us at (855) 445-4554 (TTY: 711) or fill out a contact form, our team can help you identify if your loved one qualifies for our services. All PACE participants must be:
- Age 55 or older
- Eligible for nursing home level of care as determined by the State of Michigan
- Live in the PACE Southeast Michigan service area (link)
- Be able to live safely in the community with the support of PACE Southeast Michigan services
Once a new participant enrolls, our skilled team surrounds your loved one with the care that helps them live their most independent and healthy life. It could include:
- Visits to one of our health centers for hot meals, primary care and social activities on a schedule that works for you
- Rides in one of our buses to their medical appointments
- Access to showers, hot meals, and help with house cleaning
- Medication support from our pharmacy team
- Delivery of durable medical equipment and training on how to use it
Aside from having confidence that your loved one is receiving trusted, compassionate care, our wraparound services help caregivers take needed respite and spend more time doing things you enjoy with your loved one. Together, you, your loved one and our team will work to help them live safely and reach their health goals.
