Stay in the home you love, see us for the care you need

When you enroll in PACE Southeast Michigan, our expert team works with you to develop a personalized care plan as unique as you are. It includes medical care, social care and much more in our health center and in your home.

A comprehensive lineup of older adult services that helps you stay healthier

Managing multiple medical appointments can be overwhelming, especially when they involve traveling to multiple offices. At PACE Southeast Michigan, program participants can receive care from audiologists, dentists, podiatrists and other specialty providers right in our health centers.

Instead of coordinating transportation to multiple appointments across town, participants can access many of the services they need in the same place they get their primary care and enjoy social activities. This convenient, coordinated approach saves time, reduces stress and helps ensure participants receive the care they need without unnecessary barriers. Specialty providers collaborate closely with PACE Southeast Michigan’s interdisciplinary care team, creating a more seamless experience and personalized care plan for each participant.

By bringing specialty care under one roof, we’re helping older adults focus less on navigating the health care system and more on living safely and independently in the homes and communities they love.

Reliable transportation is more than getting from one place to another. For older adults, it can mean staying connected to medical care, maintaining independence and continuing to participate in everyday life. At PACE Southeast Michigan, transportation is an essential part of our whole-person care model.

Coordinated rides to our centers and other medical appointments are part of each PACE Southeast Michigan participant’s personalized care plan. Drivers work closely with interdisciplinary care team members like primary care providers, nurses, therapists and social workers. This approach helps reduce barriers and ensures participants can access the services they need.

Our drivers often become familiar and trusted faces for participants. Friendly interactions and consistent support can make a meaningful difference, especially for aging adults who may otherwise experience isolation or difficulty traveling alone.

The care at PACE Southeast Michigan extends beyond the clinical services. Transportation support is one more way we help older adults continue living safely in the homes and communities they know and love.
