Appointing a representative
You have the right to appoint someone to act on your behalf. You can name a relative, friend, advocate, doctor or anyone else to be your appointed representative. This appointed representative may request a coverage determination on your behalf, as well as file a grievance or appeal.

Grievance process
PACE Southeast Michigan encourages our employees, vendors, participants and/or their
designated representatives to discuss any concerns or questions in an open and collaborative manner. However, we understand that some individuals may feel more comfortable sharing this information discreetly or without disclosing their identity. To support this, and to ensure our employees, vendors, participants and/or their designated representatives have a safe way to report concerns related to fraud, waste, and abuse privately, without fear of retaliation, PACE Southeast Michigan offers multiple options for submitting confidentially:
For Grievances:
Oral Submissions – Complaints can be submitted verbally by Participant/Caregiver/Designated Representative (in-person or telephonic) directly to any PACE Southeast Michigan employee. You can also call the Compliance Confidential Hotline at (833) 874-9304 or (248) 556-9304.
Writing Submissions – Complaints can be submitted in writing in four ways:
- Participant/Caregiver/Designated reps can submit complaint/suggestions directly through PACE
Southeast Michigan website, on their own, or with staff assistance using the below Improved PACE form. - Written versions of the Improve PACE form can be found at PACE centers.
- Participant/Designated reps can also submit a letter to PACE Corporate Office. 21700

Every report we receive is important, however, not every submission results in an investigation.
Due to the high volume of complaints we receive, it is not possible to contact every complainant.
However, Hotline tips are incredibly valuable, and we appreciate your efforts to help us stamp out
fraud, waste, and abuse.
