PACE Southeast Michigan is thankful to be the Detroit Tigers’ August 12 “Charity of the Game!”
PACE Southeast Michigan is a nonprofit health plan and provider that serves low-income, chronically ill aging adults 55 and older with all-inclusive health and social services to keep them independent in the community. If a participant qualifies for PACE Medicaid, all services are provided at no out-of-pocket cost.

We play as a team.
When we watch a Tigers game, we love to see great teamwork. At PACE Southeast Michigan, we work as a team to help seniors live independently. We get excited about preventing hospitalizations, watching wheelchair-bound seniors walk again, and lowering the stress family members feel as they worry about an aging loved one.
All team members play a role.
Each member of a baseball team helps a team to succeed, from the trainers to players, coaches to grounds crew. Our PACE Southeast Michigan team has a great lineup of experts too. Social workers and nurses call the plays. Physical and occupational therapists, registered dietitians, and patient care assistants help each person get stronger. PACE even has a team of drivers that take our participants to our day health centers and their medical appointments.
Keeping everyone healthy leads to success.
Just like the Tigers, we don’t want anyone on the “injured list.” We help to keep our participants healthy. We also want them to enjoy life with their family and friends. Our care teams make plans for each participant, including everything they need to stay safe in their home.
We trust coaches and care teams to call the plays.
A baseball team needs a coach who encourages, guides and motivates players. It’s the same with PACE Southeast Michigan’s team. Each participant works with a social worker and nurse that coordinates their care and plans for future needs. It all comes with a healthy dose of motivation too!
We celebrate wins!
Cheering on the Tigers to victory brings our city great joy. At PACE Southeast Michigan, we cheer on victories of the 2,400 participants who trust us for care. Our “wins” happen each day, when our participants enjoy time safely in the home and community they love.
We both give back to the community.
As a nonprofit that gives back to the community, we also share the value of community service with the Detroit Tigers. Our Mary Naber Community Giving Fund provides extra support to participants in emergency situations, and we regularly welcome teams of volunteers into our centers to connect with our participants.
