Reliable transportation is more than getting from one place to another. For older adults, it can mean staying connected to medical care, maintaining independence and continuing to participate in everyday life. At PACE Southeast Michigan, transportation is an essential part of our whole-person care model.

Coordinated rides to our centers and other medical appointments are part of each PACE Southeast Michigan participant’s personalized care plan. Drivers work closely with interdisciplinary care team members like primary care providers, nurses, therapists and social workers. This approach helps reduce barriers and ensures participants can access the services they need.

Our drivers often become familiar and trusted faces for participants. Friendly interactions and consistent support can make a meaningful difference, especially for aging adults who may otherwise experience isolation or difficulty traveling alone.

The care at PACE Southeast Michigan extends beyond the clinical services. Transportation support is one more way we help older adults continue living safely in the homes and communities they know and love.

Our team features a group of dedicated and experienced health professionals who will work with you and your caregivers to create a complete, personalized care plan designed around your medical, physical and social needs.

Your team cares for a small number of people. They get to know you, your health and your professional needs. You and your family participate and become part of the team as you collectively develop and update your plan of care and your goals in the program.

  • Center director: Oversee day-to-day operations of the day health center and ensure participants are receiving high-quality care.
  • Center manager: Works closely with you and your caregiver to ensure you receive the resources you need from our program.
  • Primary care physician: Oversees your entire health care plan.
  • Nurses: Work closely with you and your physician to provide the medical care you need.
  • Home care coordinators: Coordinate support services in your home.
  • Physical and occupational therapists: Help you stay active and meet personal fitness goals.
  • Dietitian: Makes sure your meals and dietary needs are met.
  • Social workers: Assist with everything from finding community resources to emotional support.
  • Recreational therapists: Help guide you through activities, groups and field trips.
  • Pharmacist: Makes sure all your medication works together to ensure optimal health.
  • Transportation manager: Schedules your transportation to and from our day center and all medical transportation.
