Understanding your medications

At PACE Southeast Michigan, we help seniors age on their own terms with independence and dignity, working as a team to help each of our participants. In the video below, Dr. Janis Rood, Director of Pharmacy at PACE Southeast Michigan explains the unique role of pharmacists on our team. There is a pharmacist assigned to each PACE center that has daily conversations with participants about the safety and effectiveness of medications that are prescribed.

Reviewing your medications with your provider can improve your health.

  • It reduces harm: Fewer medications reduce the risks for falls, confusion, low blood pressure, bleeding, and kidney problems.
  • It improves quality of life: Many people feel more clear, steady, and “like themselves.”
  • It prevents medication mix-ups: Less pills lowers medication mistakes.
  • Medications affect you differently over time: As you age, medicine stays in your body longer. This can cause stronger side effects and may interact with other medications.

Most importantly, it allows you and your provider to align your care with your goals.

How it works

We partner with you to make a plan that works best for your health. As we review your medications, we may recommend that taking less so that you can feel better.

Step 1 – Review: We look at every medicine and whether it still benefits you.

Step 2 – Discuss: We talk about matters most to you.

Step 3 – Plan: We choose together which medicines to adjust first.

Step 4 – Monitor: We check in regularly so you feel supported and safe.

Step 5 – Adjust: If something doesn’t feel right, we slow down or make a new plan.

Common questions

Reducing medications that aren’t working for you doesn’t mean you are “giving up.” In fact, it’s a wonderful gift you can give to yourself. But it’s normal to think:

  • “I’ve been on this for years—why stop now?”
  • “I don’t want anything taken away.”
  • “I don’t want to feel worse.”
  • “I expect the best treatment available.”

Those feelings make sense. But taking medication that no longer help can cause harm. And you deserve better than that.

Questions to ask your care team

  • “Is this medicine still helping me?”
  • “What are the risks for someone my age?”
  • “What would happen if we lowered the dose or stopped it?”
  • “Is there a safer option?”

Asking these questions is a smart thing to do for your health.
